We will be conducting the Indiana Youth Survey (INYS) to students in grades 6-12 during the week of April 1, 2024.

The INYS assesses students’ substance use, mental health, gambling, and risk and protective factors that can impact student success. The purpose of this survey is to collect data about the health behaviors of young people in our community in order to help us develop programs and interventions to improve the well-being of our students.

The survey will be conducted under the strictest confidentiality. Your child's responses will be completely anonymous and will not be linked to them in any way. Participation is voluntary, and you have the right to opt your child out of the survey by completing the opt out form below and returning it to the school.

You can also email Andy Shores (ashores@ses.k12.in.us) to let him know you do not want your student(s) to participate. Please include your name and the name of any students you are opting out in the email.
